minor bugs/quirks on ticketed event registration form

My beta tester found a few quirks with the “event” shortcode form. My guess is that they are easy fixes. 🙂

“Photo permission” checkbox shows up on 2nd and + tickets even when the option is disabled for the event.

When there’s only one ticket type, it would simplify things and save some confusion if the form skips the ticket type selector and just defaults to the ticket.

Also on 2nd and + tickets, it looks like there’s something wrong with the ticket type selector: it says “undefined” after I select it and add a 3rd or + ticket.

“Booking Contact Email” and “Booking Contact Phone” might be able to be auto-filled when the user is logged in. Additionally, these fields might make more sense at the very top instead of at the very bottom, especially if there are custom fields (i.e. all the contact info together).

A “cancel” button to reset the form?
