Add member_type_id to the Folluw-up funnel

We would love to have the ability to enter a member_type_id to limit the items returned from the Follow-up funnel.

It seems best for the shortcode [church_admin type="follow-up"] to only return users that have some follow-up option.

With all of our activity the Follow-up funnel page is very large.

I know it’s a bother but this is so key for us at the stage we are at as a church and in our use of Church Admin.
